Novelty Diploma Store Customization Form

The purchaser's name, diploma name, and choice of white or parchment paper must be entered. Also, the graduate's name, date of graduation, and your email address must be entered. All other fields are optional. Enter a value in the other fields only if you wish to change the default on the diploma. The text in the picture of the diploma is the default for all other diploma fields. You may pay via Paypal or via a check or money order. More information about payment is on the orders page.
